How can Case Managers help support your patients?

Through AkebiaCares, Case Managers provide a variety of solutions, including:

Coverage:

Conduct a full benefit investigation and return a patient summary of benefits within 1 business day from receipt of the enrollment form.

Access:

Obtain information on specific coverage rules such as a Prior Authorization (PA), Medical Exception (ME), or step edit and initiate an ePA when it is available. If it's not available, Case Managers will provide payer-specific forms, criteria, and contact information with the Summary of Benefits.

Reimbursement:

Enroll eligible patients in the Akebia commercial copay program and provide financial assistance for commercially insured patients by utilizing the copay card.

Education:

Connect you with comprehensive resources and tools, such as PA, ME and appeal guidance, forms, communication templates, and Low-Income Subsidy (LIS) eligibility information with easy instructions on how to apply. Case Managers can also connect you with the Akebia Medical Information Team for your clinical or medical inquiries.

Support:

Connect eligible patients to free medication based on financial need following a delay or denial of coverage.

AkebiaCares does not guarantee coverage and/or reimbursement for all patients. Akebia reserves the right to make eligibility determinations and to modify or discontinue the program at any time.

Financial support for patients

If your patient has Medicare, they may be eligible for Extra Help

Extra Help, also called Low-Income Subsidy (LIS), provides financial help for people with Medicare, and can be helpful if your patient has limited income and resources. It can also help pay for monthly premiums, annual deductibles, and prescription copays.*

*Patients may be eligible for LIS if they have an income of ≤150% of the Federal Poverty Level (FPL).

Patients may access additional educational and financial support through independent, third-party foundations including:

Copay assistance

Commercial copay assistance is available through AkebiaCares for eligible patients prescribed an Akebia medication.

Patients can apply to find out if they are eligible for copay assistance on AkebiaCares.com.
